The spine service provides comprehensive management of spinal injury, including Physiotherapy, Occupational Therapy, Assistive therapy, Vocational and community based rehabilitation, back pain, spinal deformities, tumours, osteoporosis etc.

Motion preserving spine surgeries including disc replacement and dynamic fixation, minimally invasive spine surgery including endoscopic disc excision, minimally invasive pedicle screw fixation and decompression, percutaneous kyphoplasty, thoracoscopic / laparoscopic fusion and advanced pain and spasticity management techniques like intrathecal baclofen pump implantation are some of the cutting edge surgical technologies practiced at the centre.

The orthopaedic service covers all orthopaedic ailments including trauma, joint diseases and replacements, oncology, paediatric orthopaedics and upper limb ailments. The team of orthopaedic surgeons are trained into each sub specialty, thus there is an expert available for each type of orthopaedic ailment.

Fellowship Types & Duration

• Sponsored Fellowship - 6 months
• Honorary Fellowship - 6 months
• Observer ship (National/International) - less than 6 months

Eligibility

• MS Orthopaedics OR
• MCh Neurosurgery

Key Objectives

• Build clinical decision-making skills in spine care
• Develop patient evaluation & surgical assessment skills
• Gain hands-on exposure to spine surgeries
• Learn research methodology & publication

Schedule

• The Fellow's role in the operating room and outpatient clinic is based on a mentorship model where training happens under the guided one-on-one supervision of the Mentor consultant. This allows for a progression of autonomy as skills progress.
• General OPD postings. - 03 days/Week. In OPD fellow is expected to evaluate and develop treatment plans for new patients
• Understand appropriate indications for surgery
• Operation Theatre postings. - 02 Days/Week. For all surgical cases fellow is expected to do Pre-operative planning and post-operative follow-up, assessment & documentation of every case to be done by the fellow.
• Fellow will typically have one day per week of research time

Clinical Responsibility

The fellow will have the following clinical responsibilities:

• Perform an accurate history and physical examination in both routine and complex surgical patients.
• Present patient information to consultants for case discussion and follow-up management.
• Work in close coordination and cooperation with other spine fellows.
• Document all pertinent clinical information in the form of clinical notes.
• Assume surgical responsibilities under supervision and guidance of the Mentor consultant.
• Communicate with other specialists including physicians, cardiologists, radiologists, physical therapists, and occupational therapists for provision of holistic care to patients.

Academic Responsibility

• Participation in clinical conferences, seminars, small discussion groups, journal clubs, and one-on-one instruction are integral parts of the fellowship in spine surgery.
• The fellow is expected to read extensively throughout the fellowship tenure about the spine, including pertinent journal articles in leading journals such as European Spine Journal, Indian Spine Journal, and Global Spine Journal.

Teaching Opportunities

• Opportunities are available for fellows to teach rotating residents and medical students, subject to management approval.

Surgical Training

• Fellows (FNB) will scrub in all types of cases related to degenerative spine, spinal trauma, deformity, infections and tumours.
• Fellows will assist in the operation theatre in procedures like discectomy, micro-discectomy, fusions, TLIF, OLIF, scoliosis correction, cervical ACDF, cervical laminectomy, cervical lateral mass fixation, thoracic laminectomy and fixation, etc.
• Fellows will be exposed to and learn the usage of all modern technologies like O-Arm navigation, robotic spine surgery and neuro-monitoring.
• The fellows will get an opportunity to do spinal instrumentation of all types.

Academic Requirements

• Research work: planning and designing a study, data collection, analysis, defending the project / study, and publishing it.
• Fellows typically have one day per week of protected research time.
• Fellows are required to complete at least one project that results in a paper in publishable format.
• Desirable to achieve one indexed publication, international.
• At least one national publication.
• Oral presentations at national conferences and attending at least one major conference.
• Articles review.
• Must participate and present in academic class held at least 4 days in a week.
• Regular presentations in regional / national spine conferences.
• Maintaining a log book of all clinical and surgical work.
• Viral markers for long term fellows who would scrub.

Evaluation

• To ensure that the fellow acquires adequate knowledge and develops the appropriate technical skills to meet program expectations, the fellow's performance is monitored carefully during the fellowship tenure.
• The fellow will be formally evaluated by his / her supervising faculty member.
• The fellow is encouraged to regularly give feedback to confirm that educational needs are being met.
• The fellow will present his / her learning to the department on completion of fellowship tenure along with a detailed case presentation, based on which the fellowship certificate will be awarded.
